Support Coordinators have a number of functions including...

Assist, guide & support participants to implement & promote understanding of plans

Search for, choose, set up and connect participants to the formal, informal, community and mainstream supports, services, therapists & activities they wish to engage in.

Help participation in the community

Monitor progress to ensure that participants make the most of their plans & work towards their goals.

Build capacity to coordinate supports independently in the future

Point of contact for anything to do with one’s NDIS plan

Help prepare for first plans, scheduled & unscheduled reviews & develop goals for the future

Assist with participant portal
-
Assist with service agreements with providers.

Resolve queries, concerns & complaints
Support coordination is assistance to strengthen a participant’s ability to connect with and coordinate informal, mainstream and funded supports, services, therapists & activities.
