Preplanning
Preplanning is a conversation with the participant to gather detailed information regarding their situation, current and potential mainstream, informal and community supports, and the impact their disability has on their life. The planning conversation explores the participant’s goals, aspirations, current life circumstances, supports, strengths and abilities to maximise achievement of goals. The planning conversation tool takes into account:

The person’s support needs, strengths, abilities and preferences for services to meet their goals.

Information about the participant’s current situation and needs under each of the support domains.

Focus on goals to enhance greater participation and independence in daily life

Give attention to vulnerabilities, safeguards and supports required to sustain informal care

Consider informal and mainstream supports in relation to the different domains

What supports may assist the participant to achieve outcomes and reach their goals.

Functional Capacity Assessments
The functional capacity assessment helps understand the participant’s support needs. The functional capacity assessment is screening tools to determine the severity of a participant’s disability and the impact it has on one’s life.

Guided Planning Questionnaire
The guided planning questions are a series of questions that assist with the development of the participant’s plan. The questions explore a participant’s support needs across multiple life domains. The responses are a key component for the development of a participant’s plan. It determines how participants manage their everyday activities, and attempt to understand a participant’s abilities, needs and current supports to help develop a plan to suit their individual needs.
